We offer Saturday appointments for your convenience, and accept most major insurances including some forms of Medicaid.

Preparation for your appointment

Scheduling Appointments

To schedule an appointment, please call or email our office. Please state the reason for your visit so we may book the most appropriate time slot for you. 

 

Canceling Appointments

If you need to cancel or reschedule your appointment, please call us as soon as possible. This will allow another patient to use your appointment time.  We request 24 hours notice for cancellation prior to your appointment to avoid a fee of $50.

 

Late Appointments

Please make every effort to be on time for your appointment.  We strive to stay on schedule to accommodate all patients.  If you arrive more than 15 minutes late for your appointment, you may be asked to wait or be rescheduled.

 

Emergencies

We will make every effort to schedule same-day appointments for our patients experiencing urgent medical problems.  After business hours, you may call the regular office phone and a physician is on call.

If you are experiencing a serious or life-threatening emergency, dial 911 or go to the nearest hospital.

 

Medical Records

You are entitled to copies of your medical records, whether for yourself or another medical provider. You will need to sign a medical records release authorization.  We will process your request in a timely manner for a fee of $1 per page.

 

Prescriptions

Prescription refills are processed only during our normal office hours.  If the office is closed, kindly wait for the next business day.

 

Payments

Co-payments are due at time of service.  At your first appointment, we kindly request that new patients complete a credit card authorization form, which may be used to cover balances of charges not paid or covered by your insurance.  We do not accept personal checks.

 

Disability Forms

Check with your company’s Human Resources Department about your eligibility and terms of disability or maternity leave.  To apply for short term disability, please complete the patient sections of your forms and bring them to our office.  We request 5 to 7 business days to complete the medical portion of the forms.  If the forms are not provided in a timely fashion, a $25 fee will be charged.
